The NHS Forth Valley Type 2 Diabetes Prevention team aims to reduce the number of people developing type 2 diabetes, through early intervention, personalised support, and community partnerships. They are keen to let more people within local communities be informed about the various services and support they offer.
They focus on:
- early detection and intervention in the community, and in Primary Care
- up skilling practitioners to be confident in person-led conversation, and managing type 2 diabetes
Their vision is to understand the priorities, values and feelings of every individual they interact with, so that they may facilitate health and social change for people at risk.
They offer tailored help for people at risk, including one to one conversations, group sessions, and signposting to relevant services. The team also provides training for professionals to support person-led care. Their work focuses on empowering individuals and addressing health inequalities across Forth Valley.
The team works with:
- people with Gestational Diabetes, or who have had Gestational Diabetes
- people with prediabetes or are at risk of developing Type 2 Diabetes
- people with Type 2 Diabetes
- groups who are at increased risk of health inequality and developing Type 2 Diabetes eg. New Scots, and people living in areas of high multiple deprivation
For people at risk, they offer:
- one to one conversations about what is important to them, the people at risk
- signposting to relevant partners for financial, food insecurity, physical and mental wellbeing and physical activity support
- group information and self-empowerment sessions, including targeted culturally appropriate education
- one to one intervention
For partners, the team offers:
- upskilling and training in person-led conversation
- nutrition training for primary care practitioners, leisure service and council staff, third sector staff and volunteers
- collaboration and partnership to offer person centred care in communities who don’t regularly access healthcare
