Adult Cancer Patients Consent Forms Survey Closing Date

FreeNational Services Scotland (NSS) is looking to collect the views of adult systemic anti-cancer treatment (SACT) patients, and has designed an online survey via Microsoft Forms, which can be completed on any smart device.
The survey is part of work to shape the future of consent forms for cancer treatments going forward, in order to make the process as suited to patients as possible.
They would like to hear from cancer therapy patients (or their family, friends, or other representatives) who have given consent to receive medicinal treatment (tablet, injection or drip) for cancer within the last 6 months under the care of NHS Scotland.
The survey is completely anonymous, with no personal details required; your answers will not be traced back to you and will not affect any treatment.
The deadline for responses is Wednesday 31st August.
