Kersiebank Community Project (KCP) is currently recruiting a Community Link Worker.
The position is 35 hours per week, with a salary of £23,000 per annum and a fixed term contract of 12 months, based in Falkirk.
Community Link Workers (CLW) offer non clinical support to patients, enabling them to set goals and overcome barriers, in order that they can take greater control of their health and wellbeing. Using “good conversations”, a CLW supports patients to identify problems and issues they are experiencing and to talk about what really matters to them. They support patients to achieve their goals by enabling them to identify and access relevant resources or services in their community. A CLW also maps local services, engaging with and developing productive relationships with these services including keeping informed of the status of existing and new services.
The successful candidate will undertake person-centred guided conversations with participants, identifying personal goals they wish to achieve and supporting them to meet them. They will also:
- support people to live well through strengthening connections between community resources and primary care
- support GP practice teams working with individuals and communities who experience socio-economic deprivation
- act as the first point of contact at GP surgeries where the service is delivered
- work independently whilst being part of a wider multi-disciplinary team within associated GP practices
Applicants should have excellent people skills, be a good listeners with a problem-solving attitude and a practical approach to meeting individual needs.
They must also have knowledge of the Grangemouth area and its assets (including services and organisations), understand the challenges faced by vulnerable people and those experiencing difficulties, have experience of Microsoft Office packages, and be able to travel independently throughout the Falkirk area, amongst others.
