Falkirk Health and Social Care Partnership are working with their partners across Forth Valley to plan future community palliative and end of care support in your local area. They are looking for personal and professional views about local palliative and end of life care options and support. Your feedback will be used to inform strategic plans, shaping support in Falkirk and across Forth Valley.
