Outside the Box are developing their new Coming Home From Hospital project, funded by Falkirk Integrated Joint Board, and are looking for input on the related information booklet.
The Coming Home From Hospital booklet will include information on
- Making the home ready for someone returning from a hospital stay
- Making changes while in hospital, from small things like better lighting, to putting in a new bathroom
- Staying with someone else for a while
- Planning a move, if this is right for the individual
The booklet will also bring together suggestions from people who have recently come home from hospital (what worked well and what they could do differently), and the good practice that service workers have learned over the years.
To fully develop the booklet, Outside the Box want to hear from those who have recently come home from hospital, their friends and families, about what worked for them, and what else would have helped.
They would also like to hear from staff who offer support to those returning home, to find out what would help them and those they work with.
(There will also be a booklet for staff working in hospitals and other places, to show how they can encourage and help the older people they know and care for.).
